DECLARATION BY PARENTS/GUARDIAN :

1) I certify that I am the bonafide guardian of the child.
2) I understand that registration does not guarantee admission. Admission is given only when a suitable vacancy exists & the child fulfils all the criteria of admission as per the school norms.
3) I hereby certify that the date of birth & spelling of the name of my child given in this form are correct to the best of my knowledge and I shall not make any request for changes in either later on.
4) Till the Birth Certificate and Transfer Certificate of the child is deposited his/her admission will be deemed provisional.
5) I understand that rendering false or misleading information or withholding correct information may disqualify my child for admission to this school.
6) In case of withdrawal of the student, I will abide by the withdrawal rules of the school.
7) I have read the school fee structure and payment schedule.
8) I have made satisfactory arrangements for remittance of school fee within due dates without waiting for a reminder from the school.
9) In case the fee is not paid as per payment schedule, I will pay it along with the surcharge as per the school rules.
10) I hereby certify that I understand the refund policy laid down by the school in case of midterm withdrawal of my ward.
11) I will furnish the complete medical record of my child required by the school authorities.
12) Having read carefully the rules, regulations & procedures laid down by the school and desirous of having my child/ward educated in Rukmani Birla Modern High School, I hereby agree to abide by them in all respects. I understand that the decision of the school management shall be final and binding on me.
13) I hereby certify that my ward and I shall follow all the rules, regulations & procedures laid down by the school from time to time.
